Education/Experience

A valid RD/RN license in good standing with the College of Dietitians of Ontario/College of Nurses of Ontario.

Experience teaching/counseling individuals and groups of people.

Compensation & Benefits

: The expected compensation for the position is $59K-$84K/annually. Actual compensation will be based on a candidate's credentials, experience and geographic location in which the position is located.

Responsibilities

Educate and advise patients on self-management skills in both individual and group settings focusing primarily on diabetes and secondarily on other health conditions such as general health & metabolic conditions (weight management & obesity).

Develop a care plan for patients and collaborate with the multidisciplinary team, as needed.

Complete detailed patient encounters within EMR system.

Teach administration of insulin effectively, including titration and management, as per medical directives (for applicable patients).

Teach self-monitoring of blood glucose (SMBG) effectively for patients living with diabetes, as per medical directives.

Perform patient follow ups in person or remotely (email, telephone, video).
