The nurse will work collaboratively with patients, family physicians and other team members to develop and implement care plans and provide follow-up with patients. The nurse will assess, manage and support patients to provide comprehensive care, education and guidance to patients with a variety of clinical conditions. Duties will vary depending on the clinic and patient population.
